Scheduling a visit

   Call the practice

   Call (708) 540-6973 during office hours. If a doctor referred you, have the physician's name ready.

   Check your referral

   Many insurance plans require a referral to see a specialist. If yours does, ask your primary care doctor for one before your visit.

3. 03

   Bring the basics

   Photo ID, your insurance card, a list of your medications, and any recent reports or imaging your other doctors gave you.
